It is offically the holiday season! The holidays often include travel, changes in schedules, and additional stress. We want to help you prepare so you can make the most of the joy the holidays bring.

Helpful Tips for the Holidays

Traveling through the holidays can cause headaches when it comes to preparations. Below, we have a few helpful tips on how to make preparations a little less stressful.

Medication Lists

It is vital to always carry a medication list with you in the case of emergencies. This list should include:

- The EXACT name of the medicaion

- The dosage (how much you take)

- The Frequency (how often you take it)

It is also important that you include the prescribing physician and preferred pharmacy for ease of contacting should additional clarification be needed.

Helpful Tips and Tricks

- Request refills in advance and shedule appointments early should they be required to obtain the prescription.

- Be mindful of how medications need to be stored (i.e. refrigerated, away from other medications, in a dark place, etc.).

- Regularly check medication expiration dates and rotate out any travel/back-up supply as needed. Most pharmacies will have a medication disposal bin available.

- Storage containers with various organization models are available to help separate out morning, noon, and evening medications. These can help keep you on track and reduce forgotten doses.

How the HumHealth App Helps

Reviewing Your Vitals

The HumHealth app plays an important role in your experience on the RPM program. A list of your vital readings is stored here for review and use by yourself and others in your care team.

Navigate to the menu button at the top right corner (3 horizontal lines)
Scroll down to the Self Assessment section, and select 'Vitals.'
A new screen will show up that looks simiar to the home page with purple icons at the top. Do not select any of these icons.
Further down on the page, select and review the specific vital you are looking for, like weight or blood pressure. These can be viewed in a list format or a graph that is adjustable with time frames.
