Getting to Know You

One of the benefits of long term care programs such as Remote Patient Monitoring, is the opportunity to build personal relationships with your Patient Care Coordinator. We would like to take the opportunity to get to know you better this month. The better we know you, the more we can provide more personalized care that takes into account your lifestyle and preferences.

 

Cooking and Eating Habits

Cooking and eating habits can play a significant role in your blood pressure readings. We want to know more about your diet!

- What kinds of things do you eat during a normal day?

- What do you like to cook?

- What is your favorite meal at a restaurant?

- Where can we make some substitutions that are better for your blood pressure?

Mindful eating does not have to mean giving up foods your enjoy, it is about learning moderation and making healthy choices.

Routines and Hobbies

Regular exercise is great not only for helping to lower your blood pressure, but it improves your overall health. However, we understand that a regular exercise routine can be difficult to maintain. Let's talk about where you get your exercise each day.

- What does your day usually look like?

- Do you have a regular exercise or walking routine?

- What kinds of hobbies do you have?

- How can we get more movement into your day?

Exercise does not have to look like exercise or going to the gym. Everyday tasks like household chores or gardening count too!

Travel Preferences

From the stress of packing for a trip to the foods you eat while on vacation, traveling can impact our blood pressure. However, there are things you can do to reduce these changes.

- What types of places do you like to visit?

- Do you pack your own snacks or do you indulge in gas station snacks?

- Do you plan and pack ahead of time, or do you fill your suitcase the morning you are supposed to leave?

- Do you check your blood pressure when you are away from home?

Traveling provides opportunities for new experiences or revisiting old favorites. Let's talk about how to manage your blood pressure while on your next adventure.
