Medication Lists

Medication lists are a very important tool for both yourself as a patient and your care team. Ensuring that those lists are accurate and up to date will allow for you to receive the appropriate care and better decision making with your doctors.

 

Why are Medication Lists Important?

An up to date medication list assists in being able to provide more accurate care for you on a regular basis. However, carrying an updated medication list on your person can provide valuable information in the event of an emergency.

This list will let medical personnel know who they need to contact if they have questions regarding your medications or need to notify your doctors about medical events.

Maintaining and communicating an accurate list to your care coordinator is important because we are not always notified if your other doctor's have made any changes. 

What Should be on Your Medication List?

Your medication list should include several key pieces of information.

- The name of the medication (ex: Furosemide, Spironolactone)

- The prescribed dose (ex: 20mg)

- How often you take the medication, specifying morning or evening is also helpful (ex: 1 time daily)

- Any special instructions associated with the medication (ex: take 1/2 tablet)

- The name and contact number for the doctor that prescribed that medication

- The name and contact information of your preferred pharmacy

When Should you Update Your Medication List?

Your medication list should be reviewed with your patient care coordinator during your monthly check in. 

Be sure to inform your care team if you have been prescribed any new medications. This is important so that we can try to avoid duplicate prescriptions. Certain medications should not be taken at the same time, an accurate list can help us avoid possible interactions.

Tell your care team when you stop taking any of your medications. There can be changes to your blood pressure or weight when medications are stopped or started. Being aware of medication changes allows us to have a better idea of why we are seeing changes in your readings and better manage your health with you.

Update your medication list with your patient care coordinator even if you are taking the same medications but the dosage has changed. Changes in dosages can have an impact on your readings. It is important to be able to make informed decisions with you about your health.
