Happy New Year! As we begin this year, I would like to start by thanking you for allowing me to be your Patient Care Coordinator (PCC) in 2024. I am looking forward to starting off this year with you. Let's take a look at last year and talk about what you would like to focus on this year.

Have you ever wondered how many days you transmitted readings? Let's give you a run down of how you did last year with some interesting things that we keep track of.

Preparing for 2025

The Remote Patient Monitoring (RPM) program is a long term program that needs some good habits in order to help your care team make the best decisions with you.

One of the most important habits that we would like to help you create or maintain, is regular communication with your PCC. There are a variety of ways to do this, let's look at some of the options:

- Monthly check-in phone call (tell your PCC what day is best for us to reach out to you)

- Send an email to [email protected]

- Send a message through the HumHealth app

The other really important habit for this program is checking your blood pressure and/or weight at least 18 days of the month. We have some tools to help with that as well:

- Your PCC would be happy to schedule daily emails to help remind you to take your readings

- We can help set up your app to send you notifications to take your readings

- We have several books we can recommend if you are interested in learning more about creating habits

Making it Happen

The first check in of the year with your Care Coordinator will be a little different than what you may be used to. We want to make sure that we have accurate information in our systems to be able to provide the best care for you. So expect to be asked a few extra questions this month!

Please be sure to have your insurance cards available, even if you have not had any changes. We want to be sure that we entered your information correctly and fix any discrepancies.

Your PCC will also be going over your medication list with you. It can be difficult to remember if you informed your providers of any changes throughout the year. We would like to take the guesswork out of it and make sure our information matches yours.

We are also going to be asking about your address, contact information, and confirming that we have the correct information so that we can make sure that we are communicating with you in the best manner.
