When reviewing your blood pressure readings from the past year with your patient care coordinator, it’s important to ask questions that help you understand patterns and next steps. Consider asking whether your readings show consistent control or concerning trends, and how they compare to recommended targets for your age and health conditions. Please remember to connect with your care coordinator every month to review your progress, ask questions, and address any concerns about your health plan. These regular check-ins help keep your care on track and allow for timely adjustments when needed. Also, be sure to take and record your blood pressure readings on at least 18 days each month. Your monitoring device is portable and travel-friendly, making it easy to stay consistent with readings at home or while you’re on the go, so your care team has the information needed to support your health.
