Patient Portal Log-in Troubleshooting
Having trouble logging in? Try these simple steps to work through log-in issues.
- In the "View Health Record" section, select the "Trouble logging in?" button that appears at the bottom right corner of the box where log-in credentials are input.
Forgot username:
- Should altering your information fail or should you need additional assistance, please feel free to contact our office and we can assist you further.
- Be advised that too many failed log-in attempts will result in your account being locked by the software. You must call our office to have it unlocked.
- Please note that there is an approximately 15 minute delay in updating information when a username or password is set or reset to receive an email.

- Select "Forgot Username?" at the top of the box.
- Input your name and date of birth as it appears on your chart at our office.
- An email will be sent to the account on file within a few minutes with the associated username.
- Click the link provided and you will be redirected to the log-in page.
- Select "Forgot Password?" at the top of the box.
- Input your username (this is usually the email we have on file).
- It will ask for a "Captcha" verification. Enter the information in the box and select submit.
- An email will be sent to the account on file within a few minutes with a link to reset the password. Click the link to be redirected.
- Enter the username previously entered and select submit.
- Due to the nature of the information within this account, it will ask for multistep verification for security purposes. Select where you would like your verification code sent. You should receive it and be able to enter it within a few minutes. These are case sensitive; be mindful of capitalization.
- Enter your password, enter the password again in the confirmation box, and select "Next" when finished. We suggest writing this down exactly as it was input for safekeeping. Passwords are case sensitive; be mindful of capitalization. You should be instantly rerouted to your portal if it was successful.
