Remote Patient Monitoring

What is Remote Patient Monitoring?

  • Physician monitoring of your trackable health data
  • Monthly communication with your provider
  • Assigned Patient Care Coordinator (PCC)
  • Alerts for out of range readings with follow up from PCC/Physician

Our remote patient monitoring program allows you and your healthcare provider to manage your health from the comfort of your own home. This is especially helpful for chronic disease management and monitoring in between scheduled appointments.  This is a great compliment to your long term healthcare and health coaching. 

Bluetooth and 4G enabled health tracking devices, such as body scales and blood pressure monitors, send important health data to your doctor’s office. Our providers use this information 
to monitor your progress and detect problems early. 

Remote Patient Monitoring is a billable service*.  Medicare and other major insurance carriers provide coverage, but check with your insurance to verify coverage.

Everything to get started is included:
  • The appropriate Bluetooth or 4G device(s) (blood pressure monitor and/or body scale)
  • Personalized training and setup
  • Mobile phone application  

Your responsibilities:
  • Take your measurements as prescribed by the Physician
  • Send a minimum of 18 readings per month
  • Communicate with your Patient Care Coordinator

*Covered by most insurances, including Medicare.  There may be some out of pocket monthly expense related to these services due to deductible or copayment/coinsurance.  Check with your insurance carrier to see if Remote Patient Monitoring is covered.

Tips and Tricks for success on RPM:
  • Change batteries and/or charge devices regularly
  • Consistency is key! Take readings around the same time of day and in the same manner each time. 
  • Monitor your phone for calls and messages to remain in communication with your care team
  • Ask questions as they arise
  • Keep your care team updated when you have changes in routines, medications, travel plans, etc.
