What is Remote Patient Monitoring (RPM)?
Our remote patient monitoring program allows you and your healthcare provider to manage your health from the comfort of your own home via bluetooth or 4G enabled health tracking devices. RPM is a compliment to long term healthcare for patients with chronic diseases; by assisting to monitor vital health data, problems can be detected between regularly scheduled appointments. This can reduce additional doctors appointments and hospitalizations.
RPM inlcudes your provider monitoring your trackable health data. To facilitate this, you will be assigned a Patient Care Coordinator (PCC). They will reach out on a monthly basis and will work with you to maintain consistent communication regarding your vital readings. You will receive additional communication for readings that are out of the designated range to ensure you are doing well. RPM is a billable service covered by most insurances.*

*Covered by most insurances, including Medicare. There may be some out of pocket monthly expense related to these services due to deductible or copayment/coinsurance. Check with your insurance carrier to see if Remote Patient Monitoring is covered.
Monthly Education
On our mission to continue empowering our patients through education, we provide our RPM patients with monthly education pieces. These are on various topics like medication management, building habits, understanding blood pressure, yoga and relaxation, and reducing sodium.

- Change batteries and/or charge devices regularly
- Consistency is key! Take readings around the same time of day and in the same manner each time (i.e. taking readings on the same arm, wearing or not wearing house shoes when taking weight, etc.)
- Monitor your phone for calls and messages to remain in communication with your care team
- Ask questions as they arise
- Keep your care team updated when you have changes in routines, medications, travel plans, contact information, etc.
